Copay Card Eligibility Requirement

Eligibility criteria:

1. This card is good for use only with a valid prescription for Enstilar® (calcipotriene and betamethasone dipropionate) Foam 0.005%/0.064, Taclonex® (calcipotriene and betamethasone dipropionate) Topical Suspension 0.005%/0.064, or Picato® (ingenol mebutate) gel 0.015%, 0.05%. 2. This card is valid for up to 12 prescription fills per calendar year for Enstilar®, Taclonex® Topical Suspension, or Picato®. 3. For patients with commercial health insurance, this card may reduce out-of-pocket expenses. 4. Offer good only in the USA at participating retail pharmacies. 5. Original card must be presented to the pharmacist at the time the prescription is filled. Not valid if reproduced. 6. Only one card per patient. 7. The selling, purchasing, trading, or counterfeiting of this card is prohibited by law. 8. Card is not transferable. 9. You must be 18 years of age or older to use this card with your Enstilar® or Picato® prescription. For Taclonex® Topical Suspension prescriptions, you must be 12 years of age or older to use this card, but a legal guardian over 18 years of age must redeem the card for you. 10. This card is not health insurance. 11. You are responsible for the use of this card and for complying with reporting obligations, if any, of your insurance plan. 12. Patient is not eligible to participate in the program or use the card: (1) if enrolled in any state or federally funded programs, including but not limited to Medicare (including Medicare Part D,) Medicaid, Medigap, VA, DOD, TriCare or CHIP, unless not eligible under such state or federal program for drug coverage, in whole or in part, or (2) where prohibited by law. 13. Offer void where prohibited by law, taxed, or restricted. 14. Not valid in combination with any other offers, discounts, or programs. 15. Card has no cash value. 16. Taclonex® Topical Suspension is for use on skin only (topical). Do not get Taclonex® Topical Suspension near or in your mouth, eyes, or vagina.

Taclonex® Topical Suspension may cause serious side effects including too much calcium in your blood or urine and/or adrenal gland problems. You may experience skin problems such as thinning of your skin, burning, inflammation, itching, irritation, dryness, changes in skin color, redness, infection, or raised bumps on your skin. Wash your hands after using Taclonex® Topical Suspension. You may get eye irritation if you accidentally get Taclonex┬« Topical Suspension in your eyes. If you accidentally get Taclonex® Topical Suspension on your face or in your eyes, wash the area with water right away. Do not bandage or cover the treated skin area.

The most common side effects are inflamed hair pores (folliculitis) and skin burning. Tell your doctor if you have a skin infection, have a calcium metabolism disorder, have erythrodermic, exfoliative, or pustular psoriasis, have thinning-skin (atrophy) at the site to be treated, are getting light therapy (phototherapy treatments) for your psoriasis, are pregnant or plan to become pregnant, or are breastfeeding or plan to breastfeed. Tell your doctor about all the medicines you take, including prescription and nonprescription medicines, vitamins and herbal supplements. Be sure to tell your doctor if you use other corticosteroid medicines and/or other medicines for your psoriasis. Do not apply Taclonex® Topical Suspension to the scalp in the 12 hours before or after any chemical treatments to your hair. Avoid spending a long time in sunlight. Avoid tanning booths and sun lamps. Talk to your doctor if you get sunburned.

Call your doctor for medical advice about side effects. You may report side effects to the FDA at 1-800-FDA-1088 (1-800-332-1088) or www.fda.gov/medwatch.

INDICATION AND USAGE

Taclonex® Topical Suspension is a prescription medicine that is to be used on the skin to treat:

  • plaque psoriasis of the scalp and body in people 18 years of age and older
  • plaque psoriasis of the scalp in children from 12 to 17 years of age

Apply Taclonex® Topical Suspension to areas affected by psoriasis 1 time a day for up to 8 weeks. You should stop treatment when your plaque psoriasis is under control, unless your doctor gives you other instructions. If you are 18 years of age or older, you should not use more than 100 grams of Taclonex® Topical Suspension in 1 week. If you are 12 to 17 years of age, you should not use more than 60 grams of Taclonex® Topical Suspension in 1 week. Use Taclonex® Topical Suspension exactly as your doctor tells you to use it.
